2023-24 Volunteer Information

Overview

Welcome!

If you plan to volunteer in your child’s classroom, in the office, for PTSO events, or as a chaperone for field trips you must complete the required volunteer paperwork listed below:

  • 1st time volunteer requirements
    • Fill out/sign all pages of the current volunteer packet (link at bottom of page).
      • If you have fingerprints taken at a Live Scan location rather than the Serna Center you do not need the principal’s signature on page 9, you just need to bring the Request for Live Scan Service page (page 7).
      • Field Trip Driver Clearance – If you want to obtain clearance, please fill out/sign pages 11 & 12 of volunteer packet.
        • Attach a copy of your driver’s license and vehicle insurance declaration.
          • If you do not wish to obtain driver clearance, you do not need to fill out or submit pages 11 & 12.
      • Current TB test results (valid for 4 years).
      • Copy of COVID vaccination card or acknowledgement that you agree to be tested on campus prior to volunteering.
      • Complete the following Volunteer Mandated Reporter Training and print the certificate you will receive at the end of the training. This must be updated every 2 years.

  • Returning volunteer requirements
    • Fill out/sign pages 1, 3, 4 & 5 of the current volunteer packet (link at bottom of page).
    • Fill out/sign driving forms (pages 11 & 12) if you intend to drive for field trips, along with a copy of your driver’s license and vehicle insurance declaration.
    • Confirm with the office that your previous TB results are still valid (results expire every 4 years).
    • Copy of COVID vaccination card or acknowledgement that you agree to be tested on campus prior to volunteering.
    • Complete the following Volunteer Mandated Reporter Training and print the certificate you will receive at the end of the training. This must be updated every 2 years.

You cannot begin volunteering prior to receiving confirmation of your clearance from the office staff.

Please print out and submit completed volunteer forms, along with copies of all the required documents at the front office. Once they have been processed, you will be contacted about approval.

Free & Voluntary On-Site COVID-19 Testing

SCUSD is pleased to offer free and voluntary COVID-19 screening testing for all students and staff. Parent/guardian consent is required to provide testing to students under 18 years old. Consent is given when registering your child for the testing program.
